I will:

* Follow all instructions given by the teacher

* Protect eyes, face, hands, & body when involving in science activities & investigations in the lab & field

* Carry out good housekeeping practices

* Know where to get help fast

* Know the location & use of the eye wash, shower, first aid, & fire extinguisher

* Conduct myself in a responsible manner at all times

* Not conduct any science activities outside class without instructions from my teacher

I, ________________________________ , have been instructed in lab & field safety and emergency techniquest needed for my science class. I have made a grade of 100% on the safety exam, which allows me to participate in lab activities. I understand & agree to follow the safety regulations set forth above & in the safety instruction I receive from my teacher. I am aware that my safety & the safety of others depends on by behavior. With this in mind, I will closely follow the oral & written instructions provided by my teacher &/or school administration.
