Instructional Assistant (IA) Application Form To apply for an Instructional Assistant (IA) position in the Department of Biology for the Spring 2010 semester. E-mail the completed form to Dr. David Lemke. DOC
Graduate Student Travel Funds Request Form To apply for funds to support travel to meetings of professional societies. Student must be presenting a paper or poster to be eligible for funding. DOC
Bio 5100/7100 Course Fee Waiver Form To request a waiver of the tuition and fees associated with Bio 5100 or Bio 7100 (Professional Development). Note that this waiver is only granted to graduate students hired as Instructional Assistants during their first three semesters of employment and that a new request must be submitted each semester. Request for Exemption from Non-Resident Tuition To request a waiver of the non-resident portion of tuition. Note that this waiver is only granted to graduate students hired as Instructional Assistants or Research Assistants who are employed at least half-time by the Department. PDF
     
Masters Committee Approval Form For departmental approval of the composition of a student's thesis committee. Must be signed by Biology Graduate Committee chair and Departmental Chair. DOC
